Alhamdulillah nasib baik Vincent Tan bagi jgk saguhati.aku dapat rs kan yg Vincent Tan ni jenis baik dan pemurah.tengok muka pun dah tau.anak2 dia pun dia X kisah kwin dgn bangsa apa.

Kiranya lepas sentakan la baru hilang kawalan ek, BamKelana..
Kalau tower x notify ttg kerja2 di runaway tp bagi permission utk landing, mmg jenuhlah MAHB ni nak menjawab. Nnt nak tanya anak akak kot dia ada source dari Subang. Kalau masih kerja di sebelah airport Subang tu cepat la dapat perkhabaran..

kebanyakan lelaki di malaya ni jahil tentang hukum agama Islam.
jika suami meninggal tanggungjawab memberi nafkah kepada balu
dan anak2nya jatuh kepada wali anak2 iaitu adik beradik lelaki simati.
wang epf simati sepatutnya untuk dibelanjakan oleh adik2 beradik
untuk anak2 dan balu simati mengikut hukum Islam. bukan untuk
disapu.

Kemalangan di LTSAAS: Kenderaan mangsa statik ketika pesawat mendarat

PUTRAJAYA: Laporan awal berhubung kes kemalangan di landasan Lapangan Terbang Sultan Abdul Aziz Shah (LTSAAS) Subang mendapati kenderaan dinaiki kakitangan Malaysia Airports Holdings Bhd (MAHB) yang maut dalam kejadian itu, berada dalam keadaan statik ketika sebuah pesawat persendirian mendarat.

Menteri Pengangkutan, Anthony Loke, berkata Perodua Kembara terbabit adalah kenderaan pengiring kepada sebuah kenderaan penyelenggaraan yang membawa tiga pekerja kontraktor lain, dan kedua-duanya masuk ke landasan lapangan terbang itu kira-kira jam 1 pagi 18 Mac lalu, bagi menjalankan kerja mengecat.



Katanya, komunikasi antara Pengawal Trafik Udara yang bertugas dengan kenderaan pengiring menggunakan walkie-talkie, manakala kenderaan kontraktor tidak ada sebarang cara komunikasi dengan menara kawalan.

Beliau berkata, pada jam 2.15 pagi, pengawal menara menerima permintaan supaya landasan berkenaan dikosongkan dan berdasarkan komunikasi ketika itu, Pengawal Trafik Udara merekodkan dalam buku log bahawa kerja penyelenggaraan di landasan sudah siap serta semua kenderaan sudah mengosongkan landasan kecuali dua kenderaan yang melakukan kerja mengecat.

“Mereka (pekerja penyelenggaraan) menggunakan kenderaan kontraktor bergerak di sepanjang garisan tengah dan kenderaan pengiring mengekori dengan rapat.

“Selepas seketika, pekerja itu sedar bahawa kenderaan pengiring tertinggal di belakang tanpa sebab yang jelas kepada mereka dan kekal statik kira-kira 30 meter di belakang,” katanya pada sidang media berhubung laporan awal kemalangan itu, di sini, hari ini.

Pada 19 Mac lalu, kakitangan MAHB, Mohd Ruzaimi Iskandar Ahmad Razali, 39, maut selepas terbabit dalam kemalangan dengan sebuah pesawat persendirian di landasan LTSAAS, sehari sebelumnya.

Loke berkata, selepas ketua pekerja kontraktor itu melihat pesawat persendirian terbabit terbang rendah untuk mendarat, mereka segera mengalihkan kenderaan mereka dari landasan tetapi kenderaan pengiring berkenaan masih tidak bergerak.

“Berdasarkan kenyataan saksi, mereka memancarkan lampu kenderaan beberapa kali kepada kenderaan pengiring itu untuk menarik perhatian, namun tiada maklum balas,” katanya.

Loke berkata, selepas pesawat itu mendarat dan berada pada kelajuan 90 hingga 100 knots, juruterbang merasakan yang pesawat itu melanggar sesuatu di landasan.

Beliau berkata, hasil laporan awal turut mendapati kru udara pesawat persendirian itu memiliki lesen dan mendarat secara sah selepas menerima kebenaran daripada Menara Kawalan Trafik Udara Subang.

“‘Break Shift’ membabitkan hanya seorang Pegawai Trafik Udara yang bertugas selepas tengah malam hingga jam 6.30 pagi. Biro Siasatan Kemalangan Udara (BSKU) akan menjalankan siasatan lanjut mengenai sistem seorang pengawal pada satu syif.

“Selain itu, dari segi komunikasi antara trafik udara dan pihak yang bekerja, siasatan menyeluruh akan dijalankan berhubung prosedur operasi standard (SOP) penggunaan walkie-talkie berbanding komunikasi radio Frekuensi Sangat Tinggi (VHF),” katanya.

Loke berkata, siasatan mendalam juga akan dilaksanakan mengenai SOP bagi kenderaan beroperasi di landasan, terutama peralatan khas diperlukan apabila kenderaan memasuki landasan seperti peralatan komunikasi radio dan lampu kecemasan berputar.

Beliau berkata, dua Pengawal Trafik Udara yang bertugas pada hari kejadian kini sedang digantung tugas sementara siasatan dijalankan, dan hasil siasatan akhir kejadian akan siap dalam masa setahun dari tarikh insiden.

“Ketika ini, masih terlalu awal untuk merumuskan bahawa ia adalah disebabkan kecuaian manusia,” katanya.

PUTRAJAYA (Bernama): Two air traffic controllers on duty during the fatal accident at the Sultan Abdul Aziz Shah Airport's runway in Subang on March 18 have been suspended pending investigations, says Anthony Loke (pic).

The two experienced traffic controllers were on duty in shift two and shift three on the day, the Transport Minister said.

"Their action, logbooks of all the things transpired in air traffic control (ATC) will be investigated internally by the CAAM (Civil Aviation Authority of Malaysia)," he said in a press conference on the preliminary report of the incident by the Air Accident Investigation Bureau (AAIB) here Friday (April 19).

In the 3.20am incident, Mohd Ruzaimi Iskandar Ahmad Razali, 39, the driver of a vehicle used to supervise maintenance work on the runway was seriously injured after it was involved in an accident with a private aircraft.



According to the report, Loke said on the day of the incident, the duty air traffic controller (shift two), allowed a vehicle to enter the threshold area of Runway 15 area at 12.50am for lighting maintenance work.

Subsequently at 1am, Loke said two vehicles comprising an escort vehicle driven by the victim and a maintenance vehicle carrying three contractor workers for runway centreline painting work were allowed by the controller to enter the runway via threshold runway 15 to carry out the painting works.

At 2.15am, the controller received a request from the lighting maintenance vehicle to vacate the runway.

"Based on this last communication, the controller (shift two) recorded in the tower logbook that maintenance works on the runway had been completed and all vehicles had vacated the runway despite there being two other vehicles still on the runway doing painting works," he said.

According to the report, he said the contractor workers doing the painting works started painting the runway centreline from threshold runway 15 and moved towards threshold runway 33.

While the contractor's vehicle moved along the centreline and was followed closely by the victim's vehicle but for no apparent reason, the victim's vehicle was left behind and remained static as far as approximately 30metres, he said.

According to the report, Loke said the controller on duty (shift two) handed over his shift at 3am to another controller (shift three) with the information that no more work on the runway was recorded in the log book.

Loke said the private aircraft, with a total of 12 persons on board from Jaipur, India, was cleared for landing after the controller (shift three) was satisfied that the runway was clear.

Loke said when the contractor's workers saw the landing light of the aircraft approaching, all three of them boarded their vehicle and drove away from the runway.

"While making a 180-degree turn, the driver realised the escorting (victim) vehicle was still static at the same last position. Based on witness statements, they flashed the headlight of the vehicle several times to attract the escort vehicle attention.

"No response was observed from the escort (victim) vehicle, and as the aircraft was getting closer to them, the driver (contractor's vehicle) drove his vehicle away from the runway," he said.

Based on Cockpit Voice Recorder (CVR) information, the report said the aircraft crew exclaimed "we hit something" and did not suggest what really happened to the aircraft.

Upon inspection of the wing of the aircraft, the leading edge of the port wing was severely damaged with the metal parts sticking to the roof of the victim's vehicle, Loke said.

Asked whether the victim was asleep, Loke said the driver's seat was reclined backwards, however, investigators had not come to any conclusion.

Asked whether the beacon on the escort's vehicle was activated during the incident, he said the specialists were still analysing the CCTV footage of the incident.

"Because it (footage) looked very pale. We can't determine whether it was put on or not fitted because the whole (vehicle's) roof was gone," he said.

To prevent such incident from happening in the future, Loke said the AAIB would further investigate the application of single controller per shift system after midnight until 6.30am.

Loke said a thorough investigation would also be conducted on the standard operation procedure (SOP) for communications between the control tower and maintenance vehicle using walkie-talkie instead of very high frequency (VHF) radio communication.

He said an in-depth investigation on the SOP for vehicles operating on the runway would be conducted especially on the need to have special equipment in the vehicles. - Bernama
